HOME CARE AND HOSPICE LIAISON

JOB SUMMARY
Perform health care institution and physician office coordination activities. Coordinate patient services activities. Coordinate health care institution and physician education activities. Coordinate community education activities. Interact with clinical leadership. Perform healthcare institution and physician office coordination activities; Establish relationships with health care institutions to facilitate and manage the transfer of patients from the health care institution to the home. Establish relationships with referring physicians to ensure written post home health/hospice services are coordinated, to include coordinating appropriate medical equipment and supplies. Establish relationships with referred patients and family members to facilitate education regarding agency services, policies and procedures following a referral, including reimbursement issues. Assist with the establishment of a home care plan as to the appropriateness of requested services, medical supplies and devices. Follow-up with referred patients/families in the hospital to ensure the company can meet the patient/family needs upon discharge. Coordinate patient services activities; Coordinate written physician orders for patients transferred from the health care institution to the company, ensuring clarification with the clinical staff in the local office. Communicate new patient admission information and/or "resume" information for patients served by another branch office to the "servicing" branch to facilitate appropriate and timely patient care. Communicate written information on "high-tech" referrals regarding special patient needs to the appropriate branch office and staff, as well as outside vendors, to ensure understanding of patient and physician needs. Facilitate and maintain constructive communication with the branch to plan and implement improved patient services. Attend mandatory branch in-service and other meetings as requested. Update the hospital activity log to facilitate understanding of patient needs by other company personnel. Educate new staff regarding the Liaison's role.
JOB REQUIREMENTS
Requires a bachelor's degree or
RN, LPN, SP, LPT, MSW.
Prefer two years' experience working with home health, hospitals, physicians, HMO's, outpatient services or social services concerning public relations or office procedures. Knowledge of Medicare/Medicaid home health regulations preferred. If licensed, successful completion of a program from an approved school of nursing, physical therapy, or speech therapy is required. The ability to coordinate the transfer of patients to home health, palliative or hospice services, resulting in uninterrupted continued medical care. Must possess a strong knowledge of post-acute services, comprehension of federal, state and ACHC guidelines. Effective communication skills (oral and written).
