Position Overview The Patient Access Navigator plays an important role in supporting Harbor Care's mission by Educate patients on services, patient rights, and use of the patient portal Provide excellent customer service in a fast-paced, high volume environment This position is responsible for answering and managing incoming patient communications in a professional and timely manner, while helping create a safe, supportive, and person-centered environment for clients, staff, and community partners. What You'll Do Key Responsibilities Answer and manage incoming patient communications in a professional and timely manner Schedule, cancel, and reschedule appointments for new and established patients Guide patients to appropriate services and programs based on their needs Accurately collect and enter patient demographic information into the electronic health record (EHR) Route messages and requests to appropriate departments and team members Educate patients on services, patient rights, and use of the patient portal Maintain strict confidentiality in accordance with HIPAA and 42 CFR Part 2 Provide excellent customer service in a fast-paced, high-volume environment Respond appropriately to urgent or crisis situations following organizational protocols Collaborate with care teams to support a seamless patient experience Participate in department meetings, trainings, and ongoing process improvements Build positive and professional relationships with clients, staff, and community partners Maintain accurate and timely documentation, records, or reporting requirements Support Harbor Care's philosophy of care, mission, and organizational values in day-to-day work Please note: Duties and responsibilities may evolve based on organizational needs, funding requirements, and program operations. What We're Looking For Required Qualifications High school diploma or equivalent Strong customer service and interpersonal communication skills Proficiency with computers and Microsoft Office applications Ability to manage multiple tasks in a fast-paced environment Strong attention to detail and data entry accuracy Ability to work both independently and collaboratively Ability to communicate effectively and work collaboratively with diverse populations Preferred Qualifications Experience in healthcare, patient access, or call center environment Familiarity with Electronic Health Records (EHR) Knowledge of Federally Qualified Health Center (FQHC) environments Experience working with diverse or underserved populations Experience in behavioral health or substance use treatment settings Bilingual in Spanish or Portuguese Experience working within nonprofit, healthcare, behavioral health, housing, human services, or community-based settings Harbor Care values diverse experiences and backgrounds.
