Job Overview:
Responsible for linking to care those who are identified through testing of infectious diseases at the Center for Comprehensive Care and Jersey City Medical Center. Assist with all functions to expand testing and linkage to care as per requirements by funding sources.
Qualifications:
Required:
Minimum Associate's Degree required Competency and proficiency with computers Familiarity with medical terminology. Experience and knowledge of infectious diseases and community resources is required Must be able to travel within
Hudson County Preferred:
Bachelor's or Master's preferred in social work or related field Bi- lingual strongly preferred
Scheduling Requirements:
Shift- Day 8:00am-4:00pm with flexibility on the shift; weekend as needed
Monday-Friday Full Time Essential Functions:
Identify and engage newly diagnosed or out-of-care individuals living with infectious diseases at JCMC, ensuring linkage to appropriate care among positive clients. Collaborate with interdepartmental staff to coordinate, facilitate, and expedite testing, linkage to care, and follow-up Create, track, and maintain accurate records, including client master lists, daily census for hospital admissions, and related patient data Maintain complete, detailed, and up-to-date documentation of patient encounters, records, and project activities in compliance with department standards Assist in diagnosing and resolving data-related issues while monitoring accuracy and compliance of information entered Monitor and evaluate project performance using established standards and tools, ensuring deliverables are achieved on-time with appropriate plans Submit on time the required reports and achieve monthly statistical targets to support program outcomes and goals Provide education to patients on infectious diseases and managed care, while educating JCMC staff and community partners on program progress and goals Refers patients to necessary social and medical services, ensuring continuity of care. Develop, implement, and maintain partnerships with other community providers to strengthen outreach and support Promote a positive and healthy workplace environment. Interact with other disciplines, departments, agencies and companies in a professional manner Performs other duties as assigned
