This role provides essential support to transplant nurse coordinators, physicians, patients, and donors throughout the pre-transplant evaluation and transplant process. As a Transplant Patient Navigator, you will help coordinate complex patient care needs, facilitate communication across the transplant team, and ensure that required clinical and administrative information is complete, accurate, and available when needed. A typical day for a Transplant Patient Navigator may include: Coordinating patient care activities under the direction of the transplant clinical team, including pre-transplant recipient and donor evaluations Coordinating patient testing, consultations, procedures, and appointments across RWJBarnabas Health sites Scheduling outpatient testing and consultations at the request of the transplant nurse coordinator and documenting corresponding actions in the medical record Reviewing clinical correspondence, including laboratory results and other test reports, and obtaining pending information for review by the appropriate RN or physician Entering transplant-related data and test results into the medical record and transplant database as directed Monitoring patient lists for compliance with program requirements, including monthly blood specimen requirements Obtaining medical records and necessary clinical information from dialysis units, referring physicians, consultants, and other providers Preparing patient charts for initial evaluations, revisits, and re-evaluations by ensuring ordered tests and required documentation are complete and available for clinical review Maintaining chart completeness by adding missing documentation such as progress notes, medication records, and other required forms, and ensuring appropriate patient identifiers are present Supporting the transplant clinical team with specific patient care needs, including follow-up phone calls, coordinating testing and procedures, obtaining medical records, and assisting with blood draws or vital signs as directed Assisting with insurance authorization and pre-certification activities related to requested services Serving as a medically knowledgeable, friendly, and responsive resource for patients and caregivers, referring providers, RWJBH providers, and staff Developing and maintaining effective working relationships with referring physicians, consultants, affiliates, dialysis units, and office staff to support seamless transplant care Managing communication for transplant services and ensuring information is appropriately communicated to patients, caregivers, providers, and members of the transplant team Maintaining and verifying source documentation to support accurate and timely patient care Assuming responsibility for the organization and integrity of patient medical records Monitoring medical records for regulatory compliance, including appropriate patient identifiers, approved abbreviations, and current patient care summaries Supporting a collaborative team environment focused on patient-centered care, quality, safety, and continuous improvement This role might be for you if: Clear, compassionate communication defines how you interact with patients, families, physicians, and colleagues You enjoy balancing clinical, administrative, and care coordination responsibilities in a fast-paced healthcare environment You are highly organized and detail-oriented, with the ability to manage multiple patient needs, appointments, records, and follow-up activities You are comfortable learning clinical information, transplant-related terminology, and program requirements You understand the importance of accurate documentation and maintaining complete, compliant medical records You demonstrate professionalism, sound judgment, responsiveness, and a commitment to patient-centered service You value collaboration and enjoy working closely with nurses, physicians, patients, caregivers, referring providers, and other members of the healthcare team Continuous learning and professional growth motivate you To be considered for this Transplant Patient Navigator opportunity: This position offers a consistent Monday through Friday 8:00am
