Patient Navigator - RWJBarnabas Health At RWJBarnabas Health, Patient Navigators play an essential role in guiding patients with substance-use disorders through the healthcare system and connecting them with the resources needed to initiate and maintain recovery. Working collaboratively with the OBAT team, healthcare providers, system partners, and community-based organizations, the Patient Navigator helps patients understand their options, address barriers to care, coordinate follow-up, and establish linkages to services that support ongoing recovery. This role contributes to a compassionate, patient-centered environment committed to high-quality care, coordination, and equitable access to services. A typical day for a Patient Navigator may include: Screening patients for substance-use disorders and providing a warm hand-off to appropriate substance-use treatment services Screening for social determinants of health and providing patient navigation services to community resources that address identified needs Educating patients on multiple pathways to recovery, treatment options, and medications for opioid use disorder (MOUD) Serving as a navigator and patient advocate to help identify and remove barriers associated with access to care and recovery Managing follow-up appointments to coordinate continuation of care and linkages to community resources Collaborating with system and community-based partners to ensure coordination of care Working alongside the OBAT team to support patients and ensure the highest quality of care Attending and representing the Institute for Prevention and Recovery at state- and county-level stakeholder meetings Performing other duties as assigned This role might be for you if: Strong communication, active listening, cooperation, and teamwork skills define how you work with patients, families, providers, and community partners You are committed to helping patients navigate complex healthcare systems and remove barriers to recovery You value patient advocacy and are comfortable connecting individuals with treatment, recovery, and community resources You have knowledge of ASAM and experience with ASAM placement criteria You possess the knowledge, skills, and experience necessary to effectively perform case management responsibilities You are committed to diversity, equity, and inclusion through continuous development and training, modeling inclusive behaviors, and proactively managing bias To be considered for this Patient Navigator opportunity, you should bring a strong commitment to supporting individuals with substance-use disorders and helping patients access coordinated, high-quality care throughout their recovery journey. This full-time position offers a 8:00am - 4:00pm schedule. A Bachelor's degree in a behavioral health profession is required; a Master's degree is preferred LCADC, LPC, LCSW, LMFT, or Licensed Psychologist credentials are preferred A minimum of three years of demonstrated experience working with substance-use disorder clients is required Experience with ASAM placement criteria and strong case management skills are preferred
