•Hybrid position (Will need to report to the main office in Parsippany occasionally for training, team meetings, equipment pick-up, etc.). Providing case management services to Supportive Services for Veteran Families Program. Assist veterans in gaining entry to VA healthcare (including mental health care) or community care if veterans are not eligible for VA healthcare. Assist veteran household members in gaining entry to healthcare (including mental healthcare). Connect veterans to VA healthcare by working with the VA Medical Center (VAMC) to facilitate enrollment. Help gather documentation and complete paperwork required for enrollment. Follow up on enrollment progress to ensure that the veteran is enrolled in VA or community healthcare services. Support veterans in identifying healthcare needs and barriers to adhering to recommended healthcare plans. Work collaboratively with healthcare teams to facilitate access to care. Assist veterans in understanding and communicating with providers to make informed decisions about healthcare. Problem-solve barriers to care (i.e. transportation, childcare). Assist veterans in utilizing available services including preventative healthcare. Provide education or create linkages for veterans to learn about wellness related topics. Documents service delivery accurately and timely while ensuring veteran confidentiality.

Qualifications:

Master's Degree in Social Work, Psychology, Counseling, or a related field. 1-year of prior experience in social services or healthcare working with veterans or the homeless required. VEVRAA Federal Contractor. Request Priority Protected Veteran Referral.
