Job Description The patient navigator RN provides patient-centered, trauma-informed medical case management to individuals across the HIV care continuum, ensuring timely access to medical services, treatment adherence, and continuous engagement in care. Also known as the RN Medical Case Manager (RNMCM), this position works as the lead of an interdisciplinary team to promote optimal clinical outcomes, viral suppression, and continuity of care.
