In addition, the Harmony Healthcare Long Island has 4 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. Senior Nurse Outreach Supervisor The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.POSITION SUMMARY
The Community Health Advocate functions as a member of an interdisciplinary team to engage patients in self-care initiatives assess for barriers to care and to provide support to patients who need assistance in navigating the health care system. Advocates for and supports the client, engages with community agencies/health care providers and others on his behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/ or hospitalizations. Provides clinical support to the Team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions, resources to maintain focus on outcomes and best practices. Responsibilities Work collaboratively with clients and their families to provide supportive services and coordinated care. Conduct monthly home visits to assess needs, provide support, and ensure continuity of care. Complete health risk assessments to identify individuals who may benefit from supportive services related to birth equity, social determinants of health, and care coordination. Connect clients to appropriate community resources and enhanced social support, including childbirth education classes, workshops, and other perinatal services. Assist clients in developing individualized birth plans and postpartum care plans. Facilitate access to parenting education and support services, including: Parenting classes or workshops Doula support services Peer support groups and clinical support groups such as postpartum support groups Breast education and support Provide culturally appropriate and understandable health education on perinatal topics using techniques such as the teach-back method to ensure comprehension and informed decision-making. Deliver patient engagement and educational programming, including printed and electronic materials, for individuals of childbearing age.Topics should be promoted:
Self-care and self-advocacy Awareness of birthing rights Effective communication with healthcare and social service providers Support efforts to improve access to early, continuous, and comprehensive prenatal and perinatal care, with emphasis on individuals at higher risk for poor birth outcomes. Ensure postpartum visits are completed in a timely manner, including monitoring, follow-up, and encouragement to support recovery, well-being, and sustained engagement in care. Promote healthy behaviors and reduce risk factors that may negatively impact birth outcomes, including preconception health education. Assist, as needed, with coordination of program activities, community events, and client referrals, in collaboration with staff and community partners. Qualifications HS diploma/GED required Bachelor's preferred in: Social Work, Psychology, Education, Rehabilitation, OT, Counseling, Community Mental Health, Sociology, etc. 2-4 years of related human services experience required in providing direct services to the underserved community of services essential to successfully living in the community, Peer with lived experience. Strong interpersonal and communication skills. This role requires flexibility to support community events, including occasional weekend work and some late-evening hours as needed. Bilingual in Spanish is required Must be willing and able to conduct home visits at clients' residences and travel to multiple locations throughout Nassau County using reliable and safe transportation.
