P M-F Care Coordinator/Patient Navigator Duties & Responsibilities:
Coordinate patient care and appointments Help patients navigate healthcare services and resources Communicate with patients, families, and healthcare providers Schedule referrals, tests, and follow-up visits Educate patients about treatment plans and available services Monitor patient progress and address care needs Maintain accurate patient records and documentation Assist with insurance, referrals, and care coordination Identify barriers to care and help resolve them Collaborate with healthcare teams to ensure quality patient care
Minimum Qualifications:
