POSITION SUMMARY
The Patient Navigator, Health Justice Mobile supports patient engagement, registration, insurance verification, care coordination, and linkage to services for individuals served through Community Healthcare Network's (CHN) Health Justice Mobile Program. The Patient Navigator works under the supervision of the Program Manager and serves as a key point of contact for patients accessing services through the mobile unit. This position is funded through the Brooklyn District Attorney's Office and continued employment is contingent upon the availability of funds.
DUTIES AND RESPONSIBILITIES
Essential Functions :
Greet, engage, and support patients accessing services through the Health Justice Mobile Program. Complete patient registration, intake, demographic updates, and required documentation in the electronic medical record. Conduct outreach and follow-up calls to encourage appointment attendance, continuity of care, and successful linkage to services. Verify health insurance eligibility and coverage and connect patients to the appropriate CHN staff for assistance with insurance-related questions and concerns. Schedule appointments, coordinate follow-up visits, and help patients connect to CHN providers and community-based services. Assist with patient flow on the mobile unit to support timely, respectful, and efficient service delivery. Provide information about available health, mental health, sexual health, and supportive services in a clear and culturally responsive manner. Maintain accurate records, protect patient confidentiality, and comply with agency policies and applicable regulatory requirements. Collaborate with the Program Manager, clinical staff, and community partners to resolve barriers to care and support patient needs. Assist with preparation of program reports by maintaining accurate patient service, referral, and follow-up information. Perform other related duties as assigned.
EDUCATION AND EXPERIENCE
High school diploma or equivalent required. Associate degree or coursework in health, human services, public health, social work, psychology, or a related field preferred. Experience working in a health care, mental health, community health, or human services setting required. Experience using an electronic medical record system, registering patients, and verifying health insurance preferred. Experience serving underserved, justice-involved, or vulnerable populations preferred.
