Ozone Park, NY Job Details Full-time $56,000 - $58,000 a year 11 hours ago Qualifications Customer communication Full Job Description Essential Duties and Responsibilities- Develop and maintain referral relationships with physicians, discharge planners, social workers, nursing facilities, hospitals, and community organizations.
- Conduct regular in-person visits to referral sources (nursing homes, doctor's office, clinics, and community organizations) to promote home care services and generate new client referrals. Identify prospective patients and referral opportunities and convert referrals into admissions.
- Meet established referral, admission, and business development goals.
- Conduct community outreach and networking activities to increase market awareness and generate new business opportunities. Track referral activity, admissions, and sales pipeline metrics.
- Attend community events, healthcare networking events, and professional meetings to develop referral relationships.
- Present and explain home care service offerings to prospective patients, families, and referral sources to facilitate enrollment and admission decisions.
- Facilitate the admission process for prospective clients and coordinate the transition from referral to enrollment. Advocate for patients to ensure appropriate services and authorizations. Develop and maintain strategic referral relationships that generate ongoing admissions and business growth. Coordinate with internal teams on referrals and authorizations.
- Maintain accurate documentation and records.
- Provide updates and reports to leadership.
- Ability to work independently in community-based settings.
