Overview of the Program Area Community Well-Being (CWB) is a cross-departmental program at InterFaith Works of CNY (IFW) designed to reach a broad and diverse population by addressing key social needs, including housing instability, food insecurity, and lack of reliable transportation. Grounded in the belief that all people deserve a chance for a happy and healthy life, CWB offers a hand up out of crisis and opens the door to stability and a stronger future. We are a community partner of the Healthy Alliance Social Care Network, providing end-to-end services to meet the health-related social care needs of Medicaid recipients in Onondaga County. Through the
NYS 1115
Medicaid Waiver, we provide screening, assessment, and referrals to appropriate services. We also offer enhanced care management and direct service provision, such as housing support and transportation to access services. Position Summary The navigator is responsible for conducting screenings, eligibility assessments, and linking people to health and social-care services. The Navigator is also responsible for ongoing care management, including transition planning. CWB provides several waiver services, and the Navigator will also be trained to directly provide those services to clients. Qualifications Bachelor's degree in a health related or human services related field is preferred. An associate's degree with relevant experience will be considered. 3 or more years of experience working in a clinical, nonprofit or managed care setting is preferred. Excellent interpersonal skills. Qualifications (continued) Excellent verbal and written communication skills. Experience working with diverse populations is preferred. Must be proficient in using technology and data systems. Must be detail-oriented with strong organizational skills. Must be able to stay calm and professional in a crisis setting. Strong phone-based communication skills with the ability to interact professionally with clients and colleagues. Essential Functions Manage incoming referrals and ensure timely outreach and follow-up. Verify eligibility and enrollment status using appropriate systems and tools. Conduct screenings using standardized tools to assess social needs. Determine eligibility for enhanced services and refer individuals to appropriate programs and resources. Maintain accurate documentation of outreach, referrals, and member interactions. Collaborate with internal teams and external partners to ensure effective service delivery. Monitor referral outcomes and escalate issues as needed. Use data tools to report trends and support program improvement. Provide feedback to leadership on challenges and opportunities for process enhancement. Participate in meetings, community events, and other job-related activities. Uphold principles of diversity, equity, and inclusion in all interactions. Stay informed about relevant policies, programs, and initiatives impacting service delivery. Responsibilities may be expanded to include other duties as assigned. Working conditions and physical demands required: The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job: Must have personal vehicle for regular travel throughout the service area (Onondaga County), must be able to travel when called upon using personal, reliable transportation, traversing streets, ascending/descending stairs. Exposure to outside weather conditions is possible. Working some nights and weekends required. Must be able to remain in a stationary position at a workstation and use a computer approximately 25-50% of the time.
