Job Description
Crossroads Health , a 501(c)(3) non-profit organization with facilities located in Lake County serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one's ability to pay. Our mission is to provide hope, healing, and healthcare to everyone. We are currently searching for a compassionate person to join our Crossroads Health team in the role of Patient Navigator-Behavioral Health. The Patient Navigator is often the first point of contact for new and returning clients. This position is responsible for responding to and screening incoming clients to identify service needs. This provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health. Most importantly, the position will: Provide trauma informed services to clients primarily on the phone and in the office. Assist clients in completing initial required documentation. Providing trauma informed individual screening, triage and care coordination to clients and their family system in an office setting as they present for walk-in or call into the intake line. Assisting in communication amongst treatment providers and with clients to ensure strong engagement and access to care.
