Quad City Area Summary:

Healthcare Consultant is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Healthcare Consultant will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration.

Requirements:

Bachelor's degree in human-services field (including, but not limited to sociology, special education, rehabilitation counseling) At least 2 years of case management experience At least 1 year of long-term care experience Proficiency in Microsoft Office such as Word, Outlook, Excel, and Teams and electronic health record experience Will travel approximately 75% of the time within the region seeing Members at home, in assisted living facilities and nursing homes to complete assessments Reliable Transportation required, eligible for mileage reimbursement For immediate consideration email your resume and job title to Keyera kwest@anchorstaffing.com
