CKP works with nephrology practices to manage and improve the health of patients with chronic kidney disease throughout South Carolina. CKP offers a statewide approach to caring for all patients with kidney disease, including chronic kidney disease, end-stage kidney disease, dialysis, and kidney transplantation. Role Description The Clinical Care Navigator is a key member of the care coordination team supporting CKP's nephrology providers and patients participating in value-based care programs. This position provides direct patient support within the nephrology practice and works closely with the CKP RN Care Coordinator, nephrologists, practice staff, and other members of the care team. The Clinical Care Navigator assists patients with navigating their plan of care, coordinates referrals and follow-up, identifies barriers to care, and supports proactive management of patients with chronic kidney disease. The Clinical Care Navigator also supports the collection, tracking, and flow of patient information and key value-based care metrics. Through direct patient engagement, care navigation, and effective use of data, this role helps ensure patients receive the right care at the right time and supports CKP's goal of improving patient outcomes.

Responsibilities:

• Individualize care coordination for patients in collaboration with and under the direction of the primary nephrologist, CKP RN Care Coordinator, and CKP leadership team.
• Conduct telephonic wellness check-ins and other patient outreach as appropriate.
• Deploy Surveys & track each patient attributed to one of our VBC programs. Maintain accurate documentation of patient outreach in applicable systems.
• Understand & facilitate the data needs of our Care Coordinators and providers, utilize Excel and other data-tracking tools to review and maintain information, and navigate spreadsheets to identify care coordination goals and priorities.
• Assist with care coordination activities under the direction of the RN Care Coordinator, including:

Patient Education Support:

Reinforce education provided by the RN Care Coordinators and help connect patients with educational resources.

Modality/Access:

Assist with coordinating appointments for access evaluation through surgical placement.

Transplant Education:

Provide patients with education and guidance on the transplant and referral process.

Appointments/Follow-up:

Assist patients with scheduling and completing recommended appointments and follow-up care.

Referrals:

Coordinate and track kidney transplant referrals and assist patients with transplant-related appointment scheduling and follow-up.

Care Team Collaboration:

Communicate and collaborate with RN Care Coordinators, providers, and other members of patient care team. Identify barriers that may interfere with a patient's ability to follow the established plan of care, assist with appropriate resources when applicable, and advocate for patient needs in collaboration with the care team. Education & Work Experience Bachelor's degree preferred; Certified Medical Assistant (CMA) credential from an accredited program also accepted Experience in a healthcare setting required Experience in care coordination, chronic disease management, nephrology, dialysis, or kidney transplantation is preferred but not required. Proficiency in Microsoft Excel and spreadsheet-based data management is required.
