The Patient Care Navigator (Social Worker) supports and advocates for the patient and the family in the pursuit of positive patient and family outcomes in multiple avenues, including physical, mental, and psychological, sociological and societal, among others, applying principles of family-centered care, empowering families, and advocating for family needs in collaboration with interdisciplinary teams and community resources. The Patient Care Facilitator (Social Worker) is a grant-funded position effective through May 2027.
Work Experience:
1 year of social work experience.License/Registration/Certification:
Must hold a current Texas Social Work license and be in good standing with the Texas State Board of Social Work Examiners. Adherence to the National Association of Social Work Code of Ethics.Education and Training:
Bachelor's degree in social work. Skills Knowledgeable of Social Work principles and standard practices. Effective oral and written communication skills. Ability to always maintain the utmost confidentiality. Knowledgeable of community resources. Able to work with people of all social, economic, and cultural backgrounds. Flexible, open-minded and adaptable to change. Demonstrated ability to connect patients and families with appropriate services.
