Job Description Description Summary:
The Patient Navigator will work with Physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge, to ensure patients and their families have the birthing experience they desire. Ensures patient is progressing towards desired outcomes by continuously assessing and responding to patient and family needs by coordinating efforts of other team members; navigating through the healthcare system; providing community resources; identifying and resolving barriers that hinder effective patient care. Provides support as the patient deems necessary.
Responsibilities:
Build relationships with physician offices. Follow and support patient throughout pregnancy; serves as a liaison for the patient and family. May assist patient with physician visits if warranted. Provide education on gestation timeline, to include milestones of fetal growth. Provide education on exercises during pregnancy. Provide information on Pediatricians in the community. Provide information on WIC and Medicaid if applicable. Assist patient with pre-registration and enrolling in prenatal classes, breastfeeding classes, and provide information on breastfeeding support groups and all breastfeeding support information to include: Breast pumps/supplies and how insurance can assist. Follow up with patient if they had any admissions during the pregnancy. Provide a tour of the Women's Services departments. Provide information for care following discharge, i.e. 2nd PKU, MD follow-up visits, etc. Perform a car seat check (must be certified by Department of Public Safety (DPS). Follow up phone calls and other duties as assigned.
