Join our dedicated team as a Care Navigator - Direct Support Professional (DSP) providing in-home services to individuals with developmental and intellectual disabilities. In this vital role, you will empower clients to achieve greater independence, enhance their quality of life, and participate actively in their communities. Your compassionate support will help clients develop essential life skills, manage daily routines, and foster social connections. No two days look the same, if you love variety and hate being stuck behind a desk, this is your role! Responsibilities Implement ISP goals and document progress each shift. Plan and lead fun, creative outings and activities in the community based on each individual's interests, from art classes and local festivals to volunteering and hobby groups. Bring your own ideas to the table, we want someone who's always got a new activity, event, or hangout spot. Build real relationships with the individuals you support by getting to know what excites them and finding ways to bring more of that into their week. Support individuals in trying new things, meeting new people, and getting more connected to their local community. Coordinate logistics for outings (transportation, scheduling, budget-friendly options) so activities actually happen, not just get talked about. Research and coordinate volunteer opportunities to support skill-building and community involvement. Complete required documentation (progress notes, incident reports as needed). Maintain detailed documentation of client progress, daily activities, and any incidents or concerns. Collaborate closely with families, caregivers, and interdisciplinary teams to ensure consistent care and support plans are followed. Qualifications High school diploma or GED required Must be at least 18 years of age Valid driver's license and reliable, insured vehicle Ability to pass a criminal background check and DBHDS-required clearances (Central Registry, sex offender registry, etc.) CPR/First Aid certification required (or willingness to obtain within [30/60] days of hire) Prior experience supporting individuals with developmental or intellectual disabilities preferred, but not required, training provided Comfortable planning and leading community-based activities and outings Basic computer literacy for documentation and shift notes Reliable, punctual, and able to work independently in a home setting We value energetic professionals eager to bring enthusiasm and compassion into every interaction.
