We're seeking a Field Care Navigator to support seniors in their homes, senior living, and memory care communities. This is a dynamic, field-based role focused on care coordination, patient support, and building strong relationships with patients, families, and care teams. This role is ideal for someone who enjoys variety, is adaptable, and thrives in a hands-on environment supporting a senior population. What You'll Do Visit patients in their homes and community settings to support care plans Assist with care coordination, follow-ups, and ongoing patient support Support chronic condition management and patient engagement Assist with telehealth visits and provider coordination Build relationships with patients, families, and community partners Support documentation, scheduling, and care team communication Participate in community outreach and engagement activities Role Expectations Travel locally to patient homes and communities (up to 1 hour radius) Daily schedules may vary based on patient needs and assignments Support a panel of patients while coordinating with assigned providers Adapt to changing priorities in a fast-paced care environment What We're Looking For Compassionate, adaptable, and reliable team player Comfortable working independently in a field-based role Strong communication and relationship-building skills Strong technical skills and comfort using EMR systems, iPads, and telehealth platforms required Medical Assistant diploma preferred Experience in patient care, senior care, or case management preferred Valid driver's license and reliable transportation required Why Join Brigade Health Join a team that values connection, dignity, and whole-person wellness.
