Job Description
Category Pharmacy - Pharmacist Job Type Contract This opportunity focuses on pharmacy professionals dedicated to transitions of care and ambulatory care services within a dynamic hospital environment. The primary responsibility is to support patients before discharge by providing thorough medication education and counseling, ensuring patients understand their medication regimens and have refills ready before leaving the hospital. This role supports a "meds to beds" approach aimed at improving medication adherence and patient health outcomes post-discharge.
Key Responsibilities:
Engage with patients to review and counsel on medication regimens prior to discharge Coordinate medication refills to ensure seamless transition from hospital to home Collaborate with healthcare teams to support the "meds to beds" program Contribute to ambulatory care initiatives within the health system's outpatient clinics and hospital units
Desired Experience and Qualifications:
Licensed Pharmacist with a focus or interest in transitions of care or ambulatory care pharmacy Preference for new PGY1 residency graduates; candidates without PGY1 will also be considered Experience in retail or outpatient pharmacy settings is welcomed Fluency in Spanish is a highly desirable asset to support diverse patient populations

The health system often utilizes contract roles to bridge staffing needs while permanent hiring processes are underway, which may take several months. to contribute to improving patient outcomes through effective medication management at a leading public health system.
