Role:
Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated and experienced PM IV Pharmacist to join our team. This is an exciting opportunity to make a meaningful impact in the lives of skilled nursing patients by ensuring safe, accurate, and timely medication management. Responsibilities:
Prepare, dispense, and verify IV admixtures and sterile compounding in compliance with
USP 797/800
standards Review and process medication orders for skilled nursing facility (SNF) patients during PM shift hours Perform drug utilization review (DUR) and clinical screening for drug interactions, allergies, and contraindications Collaborate with nursing facility staff to resolve medication-related issues and ensure continuity of care Maintain accurate patient medication profiles and documentation in pharmacy management systems Oversee and mentor pharmacy technicians during shift operations Ensure regulatory compliance with state and federal pharmacy laws and long-term care guidelines
Requirements:
Active California Pharmacist license in good standing (RPh required) Experience in IV compounding, sterile preparations, or long-term care/SNF pharmacy preferred Knowledge of
USP 797/800
guidelines and sterile compounding best practices Familiarity with skilled nursing facility (SNF) or institutional pharmacy workflows Strong clinical judgment and attention to detail in high-volume environments Ability to work independently and lead technician staff during PM shift hours Excellent communication skills for interfacing with nursing facility and healthcare teams
