Role:
Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team. Responsibilities:
Dispense and verify prescription medications accurately for skilled nursing facility residents Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness Collaborate with nursing staff and physicians to optimize patient medication regimens Monitor for drug interactions, contraindications, and adverse drug reactions Ensure compliance with state and federal pharmacy regulations and long-term care guidelines Counsel and support facility staff on medication administration and safety protocols Maintain accurate patient medication records and documentation
Requirements:
Active California Pharmacist license (RPh) in good standing Doctor of Pharmacy (PharmD) degree from an accredited institution Experience in long-term care, skilled nursing, or institutional pharmacy preferred Strong knowledge of geriatric pharmacotherapy and medication management Familiarity with California Board of Pharmacy regulations and OBRA guidelines Excellent attention to detail and strong organizational skills Effective communication and interpersonal skills for multidisciplinary collaboration
