Category Pharmacy - Pharmacist Job Type Contract This opportunity is for a pharmacist specializing in Transitions of Care within a large healthcare system serving ambulatory care and hospital settings. The role focuses on working directly with patients prior to discharge to provide medication education, counseling, and coordinating medication refills. This "meds to beds" program aims to improve patient adherence to medication regimens, reducing the risk of complications and readmissions after leaving the hospital.
Responsibilities:
Counsel patients on medication use and safety before hospital discharge Facilitate timely refills of prescriptions to support continuous therapy Collaborate with healthcare teams to ensure a smooth transition from inpatient to outpatient care Participate in ambulatory care initiatives to optimize patient health outcomes
Preferred Experience and Qualifications:
New PGY1 residency graduates are highly encouraged to apply; non-PGY1 candidates will also be considered Experience in retail or outpatient pharmacy settings is relevant and valued Fluency in Spanish is a plus, enhancing communication with diverse patient populations Demonstrated commitment to patient-centered care and medication management
