Job Description

Category Pharmacy - Pharmacist Job Type Contract This position focuses on supporting patients through the crucial Transitions of Care process within a dynamic ambulatory care setting. The pharmacist will work closely with patients prior to hospital discharge, providing essential medication education and counseling. This role aims to improve medication adherence by ensuring patients receive timely refills and understand their medication regimens before leaving the hospital—often referred to as a 'meds to beds' program. Effective patient engagement in this role helps reduce readmissions and supports long-term health outcomes. Preferred candidates include recent PGYI residency graduates, though experienced pharmacists from retail or outpatient settings will also be considered. Fluency in Spanish is a strong plus, enhancing the ability to communicate with diverse patient populations.
Location:
Van Nuys, CA (within a large county health department system serving multiple clinics and hospitals)
Key Responsibilities:
Provide medication counseling and education to patients prior to discharge Facilitate medication refills to ensure continuity of care post-discharge Collaborate with interdisciplinary teams to optimize patient medication regimens Support ambulatory and outpatient pharmacy operations focused on transitions of care Document interventions and communicate with healthcare providers as needed
Qualifications and Desired Experience:
Pharmacist licensure in California PGYI residency preferred but not required Experience in retail or outpatient pharmacy settings welcomed Spanish fluency is advantageous for patient communication Strong patient counseling and clinical skills This is a contract opportunity with the potential to convert to permanent employment following a typical hiring timeline, addressing operational needs efficiently while providing career growth potential.
