Active California Pharmacist License Minimum 2 years of acute care hospital pharmacy experience Current BLS Certification Current ACLS Certification Sterile compounding experience desired Pharmacy Residency and/or BCPS Certification preferred Oncology infusion center experience is a plus Strong clinical judgment, medication management, and communication skills New graduates are not eligible Responsibilities Review, verify, and dispense medications accurately in accordance with physician orders and hospital policies. Monitor medication therapy for safety, effectiveness, and potential drug interactions. Prepare and verify sterile compounded medications when required. Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care. Provide medication counseling and drug information to patients and healthcare staff. Ensure compliance with hospital policies, state regulations, and pharmacy best practices. Maintain accurate medication records using the Cerner electronic medical record system. Utilize Omnicell automated dispensing cabinets for medication distribution. Support pharmaceutical care for a diverse patient population, including NICU, Pediatrics, Adult, Geriatric, and Oncology patients.
