Contract Responsibilities:
Provide comprehensive ambulatory clinical pharmacy services including medication assessment, monitoring, and documentation under pharmacy CPAs. Conduct patient visits involving medication reconciliation, laboratory monitoring, patient counseling, and disease state management. Collaborate with multidisciplinary care teams to implement evidence-based medication interventions aimed at quality, safety, and cost-efficiency. Manage referrals with clear communication back to providers to ensure continuity of care. Facilitate patient access to medications through prior authorizations, benefit investigations, and financial assistance support. Oversee specialty medication protocols including initiation, monitoring, and ongoing therapy management. Serve as a resource for drug information and education to patients, families, and healthcare professionals. Precept pharmacy students, interns, and residents as assigned. Participate in program development, research, quality improvement initiatives, and committee work to advance ambulatory pharmacy services. Document interventions and outcomes within the electronic health record consistent with established standards.
Qualifications and Desired Experience:
Active Delaware Pharmacist License in good standing (or willingness to obtain). Board of Pharmacy Specialties (BPS) Certification is required in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics. Minimum of one year clinical pharmacy experience, including residency. Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD strongly preferred). Collaborative Practice Agreement experience preferred. Experience managing specialty medications and conducting prior authorizations desirable. Ambulatory care or population health pharmacy background a strong plus. PGY1 or PGY2 ambulatory care residency completion highly advantageous. Precepting experience welcomed.
