INCENTIVESfor eligible candidates! The LPG Pharmacist will be integrated into primary care with a focus on improving patient outcomes through assuring optimal prescribing practices consistent with evidence-based literature and treatment guidelines.
Conduct medication use evaluations, and support patient access and adherence to therapy.
Utilize and leverage analytic data to determine which patients will benefit from interventions, medication education and collaborate with primary care providers and specialists in the care of these patients.
Perform detailed medication reconciliation.
Conduct virtual or face to face visits to provide comprehensive medication management.
Provide recommendations to providers regarding utilization of high-cost medications.
Participate in the development of quality measures and quality improvement initiatives.
Provide educational presentations to providers.
Provide technician supervision to ensure accurate and timely submissions of prior authorizations. Strong communication and interpersonal skills, analytical skills, and leadership skills. Collaborates well in a team environment. Uses effective conflict resolution skills. Proficient in use of the EMR. Willing to serve as a preceptor to pharmacy students and residents. Certification requirements
Preferred:
Ambulatory Care Pharmacy Specialty Certification (BCACP) Minimum years of experience
Preferred:
3 years of inpatient or ambulatory pharmacy experience, or completion of PGY 1
Requirements Education:
PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).
Experience:
Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent ambulatory experience as a pharmacist or pharmacy intern for PharmD candidates.
Certification:
BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire).
License:
State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.
Other:
Strong communication and interpersonal skills, data entry skills, analytical and math skills required.
