Job Description

UF Health is seeking a Clinical Pharmacist to deliver high-quality, patient-centered medication management within our academic medical center. In this role, you will provide clinical pharmacotherapy expertise, review and verify orders, perform therapeutic drug monitoring, and participate in interdisciplinary rounds. You will educate patients, trainees, and staff, contribute to protocol development and quality initiatives, and ensure regulatory compliance. Responsibilities Provide comprehensive clinical pharmacy services, including medication therapy management and therapeutic drug monitoring. Review and verify medication orders for accuracy, safety, and compliance with evidence-based guidelines. Collaborate with physicians, nurses, and interdisciplinary teams to optimize medication therapies and patient outcomes. Participate in patient care rounds, educate patients and families on medication use and adherence. Support development and implementation of medication-related protocols, policies, and quality improvement initiatives. Precept pharmacy students or residents and contribute to staff education and training. Ensure compliance with regulatory, accreditation, and medication safety standards. Utilize electronic health records and clinical decision support tools to document and optimize care.
