Position Description Mosaic Health Center is seeking a dynamic, team-oriented, patient-centered clinical pharmacist to serve in our community. The position provides comprehensive management of our on-site pharmacy through tracking inventory and assisting with dispensing. They will also work directly with patients to optimize drug therapy and manage chronic conditions. PRIMARY DUTIES AND RESPONSIBILITIES
Duties include: Provide comprehensive medication therapy management, including medication reconciliation and comprehensive medication reviews, for patients with chronic and acute conditions Under a collaborative practice agreement with supervising physicians, adjust and manage drug therapy for chronic diseases such as diabetes and hypertension Order and interpret laboratory tests relevant to medication monitoring and therapeutic response Provide evidence-based medication recommendations aligned with clinical guidelines Manage pharmacy inventory, ordering, and clinic formulary through free and charitable and fee-for-service distributors Dispense medications as needed Deliver patient education and counseling on medication use, adherence, and self-management of chronic disease Administer immunizations Support monitoring and evaluation systems for the clinic's programs Ensure consistent and efficient use of the EHR system for improved patient care, data collection and analysis, and quality improvement projects Address social determinants of health affecting patient outcomes and medication access, including affordability and adherence barriers Participate in population health initiatives and medication-related quality improvement programs Support care gap closure and health equity efforts related to medication therapy Collaborate with a multi-disciplinary team on medication-related care plans
QUALIFICATIONS
Doctor of Pharmacy (PharmD) degree from an accredited institution Currently licensed as a pharmacist in the State of Georgia One or more years of ambulatory care or clinical pharmacy experience preferred Eligibility to practice under a collaborative practice agreement in Georgia Immunization certification and BLS certification required EHR experience mandatory
KNOWLEDGE/SKILLS
Knowledge of organizational policies, procedures, and systems Knowledge of clinic office procedures Knowledge of pharmacotherapy, drug interactions, and prescription assistance programs Knowledge of Office Suite or equivalent applications or platforms Ability to read, interpret, and apply policies and procedures Ability to communicate clearly and effectively Ability to interact with patients, medical and administrative staff, public effectively Bilingual preferred: Hindi, Urdu, Gujarati, Spanish, Tigrinya, Amharic, Oromo, Arabic, etc.
