We are looking for a
  • Critical Care Pharmacist
  • to cover the
  • Neurovascular Intensive Care Unit
  • . The
  • Neurovascular ICU
  • at •MHUMC•is an •18-bed intensive care unit•for adult patients with acute, non-traumatic illnesses and injuries to the head and spinal cord.
Common disease states/patient populations include acute ischemic stroke, spontaneous intracerebral hemorrhage, aneurysmal subarachnoid hemorrhage, cerebral edema and intracranial pressure management, ventriculitis/meningitis, encephalitis, status epilepticus, sodium disorders, brain tumors, etc. Common interventions include mechanical thrombectomy, intracranial stenting, craniotomy/craniectomy, aneurysmal coiling and clipping, ventriculostomy and other neurosurgical drain placements, tumor resection, continuous EEG monitoring, laminectomy and decompression, and transsphenoidal adenoma resection. There is no single team that manages all patients in the neurovascular ICU and predominantly includes pulmonary critical care medicine, neurosurgery, neurology, and trauma/surgical critical care. To maintain continuity of care with the various services/consults, one of the pharmacist's main roles is active participation in multidisciplinary rounds daily with the pulmonary critical care medicine service. Multidisciplinary rounds consist of an intensivist, advanced practice provider (NP or PA), pharmacist, respiratory therapist, dietician, case manager, bedside nurse, nurse manager or charge nurse, and Physical Medicine and Rehabilitation (PM&R) physician. The
  • Critical Care Pharmacist
  • provides pharmaceutical care including computer order entry, drug distribution, evaluation of medication orders for appropriateness of drug, dose, frequency and adequacy of monitoring, evaluation of drug interactions and allergies and therapeutic monitoring of drugs to patients across our adult ICUs. Our pharmacists are valued members of daily multidisciplinary rounds in the ICUs.
  • What you will do in this role:
  • Actively participate in multidisciplinary team rounds and teaching rounds to make pharmacotherapy recommendations
  • Coordinate drug distribution activities in areas under their supervision. Includes supervision of pharmacy technicians, order verification of medication orders, resolution of medication related problems and preparation of parenteral products
  • You will perform pharmacy clinical activities including but not limited to providing pharmacokinetic consults, monitoring targeted drugs, antimicrobial therapy recommendations, and evaluating appropriateness of drug therapy based on, disease state, host factors, and cost effectiveness.
  • Serve as a source of drug information to medical and nursing staffs.
  • Perform patient care unit inspections as required to meet regulatory requirements.
  • Work to minimize errors in the medication use process by utilizing S.T.A.R. (Stop, Think, Act and Review) and other error prevention techniques, especially when dealing with a situation that is out of policy, procedure, or plan, or any other high-risk situation. Strictly follow special handling procedures related to high-risk, high-alert medications and patient populations.
  • What qualifications you will need:
  • Clear and active Georgia pharmacist license
  • Graduate of ACPE-accredited School of Pharmacy is required; PharmD degree is preferred
  • PGY-2 in Critical Care and/or extensive critical care experience with Board Certification
