C. Lewis Primary Health Care Center is a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. The Health Center makes comprehensive primary care, integrated behavioral health care, oral health care, and inpatient respite care services available to all persons, including the homeless and low-income uninsured and underinsured, regardless of ability to pay. The J.C. Lewis Primary Health Care Center excels in the provision of compassionate care and promotes healing through healthy living. Position Overview:
J.C. Lewis Primary Health Care Center is seeking to add a PRN Pharmacist to our existing interdisciplinary team. The PRN Pharmacist functions as an active member of the interdisciplinary clinical team. The PRN Pharmacist acts to ensure safe, efficacious, rational and cost-effective medication therapy for the patients of the Health Center.
Primary Responsibilities:
Reviews prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provides information and advice regarding drug interactions, side effects, dosage, and proper medication storage. Assesses the identity, strength, or purity of medications. Compounds and dispense medications as prescribed by doctors and dentists by calculating, weighing, measuring, and mixing ingredients, or overseeing these activities. Maintains records such as pharmacy files, patient profiles, charge system files, inventories, control records for radioactive nuclei, and registries of poisons, narcotics, and controlled drugs. Plans, implements, and maintains procedures for mixing, packaging, and labeling pharmaceuticals according to policy and legal requirements to ensure quality, security, and proper disposal.
Qualifications:
Graduation from an accredited Pharmacy School is required PharmD or equivalent experience Licensure in the State of Georgia or eligibility is required Basic knowledge and proficiency of QS1 or other medication data system, with any additional computer skills a plus Ability to establish and maintain relationships with the medical community Ability to establish and maintain professional relationship with patients Good oral and written communication skills
