In Touch Pharmaceuticals is growing, and we are seeking a licensed Illinois Pharmacist to join our team as a Long-Term Care Consultant Pharmacist serving facilities throughout Illinois. What You'll Do As a Long-Term Care Consultant Pharmacist , you will serve as a trusted clinical pharmacy resource for our facility partners.
Responsibilities include:
Conducting monthly medication regimen reviews for assigned residents. Evaluating medication therapy for appropriateness, dosage, interactions, contraindications, route of administration, and other clinical considerations. Identifying medication-related concerns and providing recommendations to physicians, nursing leadership, and facility teams. Providing required written reports of clinical findings and recommendations. Collaborating with physicians, nurses, administrators, pharmacy staff, and other members of the healthcare team. Supporting facilities with pharmacy-related regulatory compliance and survey preparedness. Maintaining accurate and timely documentation of clinical recommendations and communications. Staying current with Illinois and federal regulations governing pharmacy services in long-term care. Serving as a responsive and knowledgeable pharmacy resource for our facility partners. Promoting safe, effective, and appropriate medication use for long-term care residents. What We're Looking For Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited school or college of pharmacy. Active Illinois Pharmacist license required. Dual Illinois/Indiana pharmacist licensure preferred. Long-term care, consultant pharmacy, or clinical pharmacy experience preferred. IV clinical experience preferred. Knowledge of medication regimen review and formulary management preferred. Strong clinical judgment and attention to detail. Excellent communication and customer service skills. Ability to build strong relationships with facility leadership and healthcare professionals. Ability to work independently while remaining an engaged member of the pharmacy and facility care teams. Flexibility to meet facility needs, including occasional responsibilities outside traditional business hours.
