Job Description

Category Pharmacy - Pharmacist Job Type Contract This position requires a skilled pharmacist to support medication management in a long-term acute care (LTAC) setting. The role begins immediately and runs for 13 weeks. The ideal candidate will have prior hospital or LTAC experience and be proficient in EPIC system use.
Responsibilities include:
Conducting medication reconciliation for discharge medication lists Gathering accurate home medication information from patients, families, or pharmacies prior to admission Providing clear discharge counseling to patients and families Applying clinical expertise in antimicrobial stewardship, including interpreting culture results and offering recommendations for infections such as UTI, pneumonia, and SSTI Managing and monitoring pharmacokinetic dosing and anticoagulation therapy Qualifications /
Desired Experience:
Licensed Pharmacist with prior experience in hospital or LTAC settings Proficiency in EPIC electronic health record system Strong communication skills for effective patient and family interactions Knowledge of antimicrobial stewardship principles BLS certification required
