Job Description

Category Pharmacy - Pharmacist Job Type Contract This opportunity is for a Staff Pharmacist position within a Long-Term Acute Care (LTAC) setting. The role offers a 13-week contract starting immediately, ideal for pharmacists experienced in hospital or LTAC environments. Candidates with strong EPIC system knowledge and medication reconciliation expertise will be highly valued.
Key Responsibilities:
Perform medication reconciliation for discharge medication lists to ensure accuracy and safety. Collect and verify patients' prior to admission home medication lists by communicating effectively with patients, families, and pharmacies. Provide discharge counseling, educating patients and their families on medication regimens and safety. Support Antimicrobial Stewardship programs by interpreting culture results and making clinical recommendations for infections such as urinary tract infections (UTI), pneumonia, and skin and soft tissue infections (SSTI). Manage and monitor pharmacokinetic dosing, including anticoagulation therapy to optimize patient outcomes.
Qualifications and Desired Experience:
Licensed pharmacist with prior hospital or LTAC experience. Proficiency with the EPIC electronic health records system. Strong clinical knowledge in antimicrobial stewardship and dosing management. Excellent communication skills for gathering accurate medication histories and providing patient education. Basic Life Support (BLS) certification required.
