Category Pharmacy - Pharmacist Job Type Contract Join a healthcare setting specializing in Long-Term Acute Care (LTAC) as a Staff Pharmacist dedicated to enhancing patient outcomes through expert medication management. This 13-week contract opportunity requires an immediate start and offers a dynamic environment where clinical expertise and communication skills are paramount.
Key Responsibilities:
Perform medication reconciliation focusing on accurate discharge medication lists. Collaborate with patients, families, and pharmacies to gather comprehensive home medication histories prior to admission. Provide discharge counseling to ensure patient and family understanding of medication regimens. Apply clinical knowledge in antimicrobial stewardship by interpreting culture results and making informed recommendations for conditions such as urinary tract infections, pneumonia, and skin and soft tissue infections. Manage pharmacokinetic dosing, including anticoagulation therapy, with ongoing monitoring and adjustments. Utilize EPIC electronic health records to document and support clinical decisions.
Qualifications and Desired Experience:
Previous hospital or LTAC pharmacy experience is essential. Proficient in EPIC systems. Strong clinical understanding of antimicrobial stewardship and pharmacokinetic dosing. Effective communication skills to facilitate collaboration with multidisciplinary teams and patients. Current Basic Life Support (BLS) certification.
