Job Description

We are seeking an experienced, dedicated, and highly skilled Full-Time Pharmacist to join our healthcare team. The ideal candidate will have a strong clinical background, extensive knowledge of pharmacy operations, and a commitment to delivering exceptional patient care. This position offers the opportunity to play a vital role in improving patient outcomes through accurate medication management, clinical expertise, and collaboration with multidisciplinary healthcare teams. Candidates must have prior pharmacist experience in a hospital, long-term care (LTC), retail, specialty, or clinical pharmacy setting. Experience with medication therapy management, patient counseling, and regulatory compliance is required. Key Responsibilities Accurately dispense prescription medications while ensuring compliance with physician orders, pharmacy policies, and state and federal regulations. Review medication orders for accuracy, appropriateness, potential drug interactions, allergies, and contraindications. Conduct comprehensive patient assessments and provide expert medication counseling regarding dosage, administration, side effects, and adherence. Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy and patient treatment plans. Administer immunizations and provide preventative healthcare services as permitted by state regulations. Manage medication inventory, including ordering, storage, controlled substance monitoring, and inventory reconciliation. Prepare and compound medications as required, utilizing proper aseptic and sterile compounding techniques. Supervise and support pharmacy technicians and ancillary staff to ensure efficient pharmacy operations. Utilize pharmacy management software, EMR/EHR systems, and electronic prescribing platforms to maintain accurate patient records and documentation. Ensure strict compliance with HIPAA, pharmacy laws, accreditation standards, and patient safety protocols. Participate in quality improvement initiatives, medication safety programs, and interdisciplinary healthcare meetings. Support specialized patient populations, including acute care, critical care, pediatric, geriatric, and long-term care patients as applicable. Required Qualifications Active and unrestricted Pharmacist license in the applicable state. Minimum of 2 years of pharmacist experience required; hospital, LTC, clinical, specialty, or retail pharmacy experience preferred. Strong knowledge of pharmacology, medication therapy management, drug utilization review, and patient counseling. Experience working with pharmacy software systems, EMR/EHR platforms, and inventory management systems. Excellent understanding of state and federal pharmacy regulations, including HIPAA compliance. Strong communication, organizational, and problem-solving skills. Ability to work independently while collaborating effectively within a multidisciplinary healthcare team. Commitment to patient-centered care, safety, and continuous professional development. Preferred Qualifications Experience in long-term care (LTC), hospital, acute care, critical care, specialty pharmacy, or pediatric settings.
