EH DC Job Description:
In collaboration with the patient and the patients' health care team, the pharmacist clinician works to optimize the patient's drug therapy to improve the quality of life and health outcomes of the patient. The standard of care includes ensuring the patient's medications are appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. Position requires high level of clinical skills, and the ability to build positive relationships with patients and colleagues, including other health care providers. This specialized role aligns with our established ambulatory clinical pharmacy practices with an emphasis in cardiometabolic conditions, including diabetes and hypertension mediation management.
Education Qualifications:
Educational Requirements:
Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction Completion of a PGY1 or PGY2 ambulatory care residency or comparable experience in ambulatory clinical pharmacy
Preferred Qualifications:
Completion of an ACPE accredited Medication Therapy Management training program Recent clinical practice, reflective of scope of this role within the previous 12 months
Licensure/Certification Qualifications:
Certification/Licensure Required:
Applicants must be board certified or become board certified within one year of eligibility of an acceptable board specialty certification including: BCACP, BCPS, or BCGP Active license to practice pharmacy within the states of MN and WI Ambulatory
  • Immunization Delivery Certification completed within 1 year from hire date Ambulatory
  • BLS certified within 90 days from hire date
