Case-by-case Job Summary We are seeking a Clinical Pharmacist to provide comprehensive pharmaceutical services for hospital inpatients. This role focuses on safe and effective medication therapy, clinical pharmacy services, and collaboration with medical staff and the healthcare team. Responsibilities
- Interpret medication orders and enter them into the pharmacy computer system.
- Accurately compound and dispense pharmaceuticals.
- Monitor drug therapy for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriate dosing.
- Recommend appropriate and cost-effective therapeutic medication alternatives.
- Support the hospital formulary and promote rational drug therapy selection.
- Provide clinical services including drug consultations, nutrition support, pharmacokinetics, anticoagulant monitoring, and pain management.
- Support IV-to-PO conversion and renal dosing programs.
- Complete medication error and adverse drug reaction reports.
- Provide oversight of pharmacy staff schedules.
- Support safe and effective medication use throughout the hospital. Qualifications
- B.S. or PharmD from an ACPE-accredited School of Pharmacy .
- Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
- Hospital pharmacy experience preferred but not required .
- New graduates and retail pharmacy candidates will be considered.
- PGY1 Pharmacy Residency/General Practice Residency or Fellowship highly preferred.
- BCPS certification preferred. Benefits
