In this role, you will verify and optimize medication therapies, provide clinical consults, and collaborate with interdisciplinary teams to ensure safe, evidence-based care. Responsibilities include medication reconciliation, therapeutic drug monitoring, patient counseling, and participation in antimicrobial stewardship and quality initiatives. Responsibilities Provide clinical pharmacy services including medication therapy management and optimization Review and verify medication orders for safety, accuracy, and appropriateness Collaborate with physicians, nurses, and care teams on evidence based medication plans Participate in patient counseling, education, and discharge medication reconciliation Monitor patient response to drug therapy and adjust regimens per protocols Support antimicrobial stewardship and formulary management initiatives Ensure compliance with regulatory, accreditation, and safety standards Precept and mentor pharmacy students or residents as assigned Required Skills Clinical pharmacotherapy Medication therapy management (MTM) Order verification and medication safety Therapeutic drug monitoring (e.g., vancomycin, anticoagulants) Drug information and evidence-based practice Electronic health record (EHR) and CPOE proficiency Antimicrobial stewardship Medication reconciliation and discharge counseling Regulatory and accreditation compliance (e.g., Joint Commission) Interdisciplinary team collaboration and communication
