Works with members of the health care team to provide safe and effective drug therapy. Serves as a drug information resource to pharmacy, nursing, and medical staffs as well as to patients. Key Responsibilities Clinical Patient Care Review and evaluate medication orders to ensure safe, effective, and appropriate therapy.
Assess patient conditions and medication regimens to identify opportunities for optimization.
Develop individualized medication treatment plans while minimizing adverse effects and drug interactions.
Monitor drug therapy through chart reviews, patient profiles, and collaboration with healthcare providers.
Evaluate treatment outcomes and recommend adjustments as needed.
Provide medication therapy recommendations regarding dosing, scheduling, route, contraindications, and potential interactions.
Serve as a clinical resource for providers, nursing staff, and patients.
Collaboration & Consultation Partner with physicians, nurses, and other healthcare professionals to achieve optimal patient outcomes.
Provide expert guidance on medication selection, administration, formulary alternatives, side effects, and therapeutic monitoring.
Maintain strong interdisciplinary relationships that support exceptional patient care.
Education & Mentorship Precept pharmacy students and residents.
Present educational programs, lectures, and clinical in-services.
Support staff competency development and continuing education initiatives.
Maintain current clinical knowledge and licensure requirements.
Quality & Operational Excellence Participate in medication-use evaluations, quality assurance activities, adverse drug reaction reporting, and policy reviews.
Assist with investigational protocols and departmental initiatives.
Ensure compliance with regulatory, licensing, accreditation, and organizational standards.
Complete required medication inspections, floor stock reviews, safety checks, and competency requirements.
Qualifications Required Education Bachelor of Science (BS) in Pharmacy or
Doctor of Pharmacy (PharmD) from an accredited school of pharmacy
Required Licensure Current Pharmacist license in the state of practice
Preferred Qualifications Two or more years of clinical pharmacy experience
PGY-1 Residency preferred
Technology Skills Experience with: Epic or similar pharmacy information systems
Clinical documentation platforms
Micromedex and other drug information resources
Pharmacy automation systems including Omnicell, McKesson Automation, ScriptPro, Baxa, and related technologies
