Job Description
Overview Core LTC Pharmacy is growing and looking for a staff pharmacist, to join our team in providing top-notch service for our long-term care facilities. As a part of Core LTC, you will help to ensure the delivery of safe, efficient, and high-quality pharmacy services to the residents we serve. We are seeking an individual who is passionate about improving patient outcomes through excellent service and leadership. Responsibilities Review and interpret physician orders for medication therapy to ensure accuracy and appropriateness. Dispense medications accurately and provide counseling to patients regarding their prescriptions. Monitor patient medication therapies for efficacy, safety, and adherence. Collaborate with healthcare professionals to optimize patient care plans. Maintain inventory management by overseeing stock levels, ordering medications, and ensuring proper storage conditions. Conduct medication therapy management reviews and provide education on drug interactions and side effects. Ensure compliance with all regulatory requirements Requirements Doctor of Pharmacy (Pharm.
