Job Description

Population Health Pharmacist Position Summary The Population Health Clinical Pharmacist is responsible for participating in Hunterdon Healthcare population health initiatives by analyzing specific data and metrics for each of our outpatient practices. This position is expected to work in collaboration with other clinical pharmacists and members of the health-care team as appropriate. This position is also responsible for educating pharmacists in training and other health care professionals. Primary Position Responsibilities 1. Population Health Initiatives. 2. Additional clinical activities related to the ambulatory care setting. 3. Precepting pharmacists in training. 4. Participates in other duties as assigned.
Qualifications Minimum Education:
Required:
Doctor of Pharmacy Preferred:
Board Certified Pharmacist Minimum Years of Experience (Amount, Type and Variation):
Required:
Completion of PGY1 residency or Board Certified Pharmacist or 3 years population health experience with significant clinical experience in health-systems or ambulatory care
Preferred:
PGY2 residency; 5 years population health experience with significant clinical experience in health-systems or ambulatory care
License, Registry or Certification :
Required:
New Jersey Pharmacist License Preferred:
Board Certified Pharmacist (e.g.
BCPS, BCACP
) or Additional Certifications (e.g.
CGP, CDE, MTM
) Knowledge, Skills and/or
Abilities :
Required:
Pharmacy skills serving patients of diverse age and disease states; Competence in required computer programs (clinical system, ADC, parenteral documentation), MS Office Proficient; Teaching/Precepting experience; Effective Verbal and Written Communication; Work Well with Team;
