Job Description

Description We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Full-Time, 40 hours per week, Mon-Fri 8:00am-5:30pm

New Providence, NJ Requirements B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate Active registered pharmacy license in the state of Kansas and additional states as required without restrictions 1 year of hospital and home infusion therapy experience preferred Two years of recent clinical pharmacy experience preferred Thorough knowledge of infusion therapies preferred Recent inventory management experience preferred Accuracy in mathematical calculations including pharmacokinetic dosing preferred Knowledge of CPR+ software preferred Job Responsibilities, included but not limited to: Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness Monitor the patient's progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. Most patient contact will be on the phone, and, on occasion, you will meet them in our infusion suite Directly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging, and labeling of compounded sterile product Assist in training and orientation of new employees, performance improvement activities and educational programs Additional licensure may be required in multi-state service areas Go in to IV room and mix medications if needed Other duties assigned Physical Demands The physical requirements listed here indicate what an employee must meet to effectively perform this role's essential functions. The employee frequently needs to communicate verbally, listen attentively, and spend prolonged periods sitting at a desk and working on a computer.
