Job Overview:
Clinical Pharmacist Specialists have advanced training in specific pharmacy practice areas or disease states to proficiently provide direct patient-centered care, manage complex medication therapies and integrated pharmacy operational services in a decentralized practice setting with an interdisciplinary care team. Clinical Services may include incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacist Specialists serve as departmental resources and liaisons to other departments. As a subject matter expert, they support both interprofessional education and education and training of pharmacists, pharmacy residents, and pharmacy students. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
Requirements:
Education, Years of Experience, Knowledge, Skills and Abilities Required:
1. Graduate of an ACPE accredited College of Pharmacy or FPGEC certification and internship hours 2. Completion of ASHP accredited PGY-1 Pharmacy Residency OR the combination of Board of Pharmacy Specialties Certification (within 1 year of hire) plus a minimum of 5 years of relevant experience 3. Active unrestricted New Jersey Pharmacy License upon date of hire. 4. Significant knowledge and expertise in advanced pharmacy practice, specialized medication therapy management, and specialty evidence-based medicine in area appropriate for clinical assignments
Education, Years of Experience, Knowledge, Skills and Abilities Preferred:
1. Doctor of Pharmacy or other advanced degree 2. Completion of PGY-2 Specialized Pharmacy Residency or relevant clinical pharmacy experience (3 years) in a hospital setting 3.
Board of Pharmacy Specialties Certification Essential Functions:
Actively participates in governance, teaching, education, and monitoring of students, residents, and clinicians. Adheres to RWJBH Organizational competencies and standards of behavior as well as departmental specific standards of practice. Assists with the formulary management service that incorporates evidenced based care, safety, and system alignment to provide optimal medication therapy while promoting financial stewardship. Develop and implement evidenced based quality improvement initiatives, research, guidelines, and/or policies as a member of the multidisciplinary patient care team Other duties and/or projects as assigned. Participate as a representative on committees, system collaboratives, or service line meetings as a liaison for the pharmacy department or facility. Provide patient-centered care and integrated pharmacy services in a decentralized practice setting with providers, nurses, and other hospital personnel.
