Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire This opportunity is for a skilled clinical pharmacist specializing in home infusion therapy. This direct hire position offers a Monday through Friday schedule from 8:00 AM to 4:30 PM, with participation in an on-call rotation. The role centers on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, serving as a vital member of a multidisciplinary healthcare team.
Key Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in compliance with sterile compounding standards and regulatory guidelines. Monitor lab values, perform pharmacokinetic dosing calculations, and adjust Total Parenteral Nutrition (TPN) treatments as needed. Provide detailed counseling and education to patients and caregivers regarding medication use, infusion pump operation, sterile techniques, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and internal departments to ensure coordinated, effective patient care and reduce hospital readmissions. Maintain adherence to State Board of Pharmacy regulations and standards set by HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigeration requirements, and ensure timely, secure shipping of medications and supplies to patient homes.
Qualifications & Desired Experience:
Minimum of 2 years of experience in infusion specialty pharmacy. Experience with IVIg therapy is highly advantageous. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and active NY pharmacist license in good standing.
