This direct hire role involves preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications while serving as a key clinical resource for multidisciplinary care teams. The position emphasizes patient-centered care through education, clinical monitoring, and close collaboration with healthcare providers.
Schedule/Hours:
Monday to Friday, 8:00 AM to 4:30 PM Participate in on-call rotation
Key Responsibilities:
Prepare specialty pharmaceuticals and IV solutions under sterile conditions compliant with state and federal regulations Monitor lab values and calculate pharmacokinetic dosing, including therapies like vancomycin and Total Parenteral Nutrition (TPN) Provide detailed patient and caregiver education on medication administration, infusion pump use, and catheter care in home settings Coordinate care with physicians, home health nurses, and pharmacy operations to optimize therapy and prevent hospital readmissions Ensure regulatory compliance with pharmacy standards including HIPAA, DEA, OSHA, Medicare, and accreditation bodies Manage pharmacy inventory with oversight of refrigerated products and prepare medication shipments securely and timely
Qualifications/Desired Experience:
Minimum of 2 years' experience in infusion specialty pharmacy Experience with IV immunoglobulin (IVIg) therapies preferred Familiarity with specialty therapy payer requirements, reimbursement procedures, and contract obligations Current and valid New York pharmacist license in good standing Proficiency in CareTend or similar pharmacy management systems This direct hire position offers the opportunity to contribute clinical expertise in a home infusion setting, enhancing patient outcomes through dedicated pharmacist involvement.
