2 Job Summary We are seeking experienced Hospital Pharmacists to join a hospital pharmacy team in Brooklyn, New York. Under the general supervision of the Director of Pharmacy and/or Pharmacy Supervisor, the Pharmacist will compound and dispense medications and pharmaceutical supplies in accordance with physician orders, hospital policies, and applicable New York State and federal regulations. Responsibilities Compound and dispense medications and pharmaceutical supplies to patients and healthcare professionals. Interpret and evaluate physician orders and prepare, calculate, and dispense appropriate medications and dosage forms. Communicate with physicians and nursing personnel to clarify unclear, incomplete, or questionable orders. Recommend changes in drug therapy when appropriate. Document pharmacy interventions and ensure identified issues are resolved. Prepare medication labels and dispense medications throughout the hospital in accordance with applicable policies and regulations. Enter medication orders accurately into the pharmacy computer system. Review medication profiles for allergies, food/drug interactions, drug/drug interactions, and therapeutic duplications. Provide medication information to physicians, nurses, and patients, including therapeutic actions, interactions, and potential adverse reactions. Compound and label IV admixtures, chemotherapy, and parenteral nutrition solutions according to established procedures. Supervise and verify the work of pharmacy technicians and aides in sterile and non-sterile medication areas. Dispense controlled substances in accordance with applicable regulations. Assist with orientation and training of new personnel. Participate in required educational programs. Perform and document monthly medication-area inspections. Work across all service areas of the pharmacy department. Apply appropriate knowledge of patient age when calculating doses for neonatal, pediatric, adult, and geriatric patients. Participate in quality assurance and performance improvement activities. Attend required committee meetings and departmental functions. Perform other duties necessary to support optimal patient care services.Qualifications Required Education:
Bachelor's degree in Pharmacy.Required Experience:
Minimum of 1 year of hospital pharmacy experience .Required License:
Active New York State Pharmacy license and current registration .Additional Skills:
Strong verbal and written English communication skills. Computer and word-processing proficiency. Ability to work effectively in a hospital pharmacy environment. Physical Requirements The position involves standing, walking, sitting, talking, hearing, and bending. Some repetitive motions may be required.
