Category Pharmacy - Clinical Pharmacist Job Type Direct Hire This direct hire opportunity seeks a Clinical Pharmacist specialized in home infusion therapies to provide expert pharmaceutical care in a dynamic setting. The role focuses on managing complex intravenous (IV) and subcutaneous medications, ensuring patient safety, and facilitating optimal therapeutic outcomes.
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to state and federal regulations. Monitor clinical parameters such as lab values and pharmacokinetic dosing (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) treatments accordingly. Deliver comprehensive education to patients and caregivers on the use of infusion pumps, sterile techniques, and catheter maintenance. Collaborate with physicians, home health nurses, and multidisciplinary teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to ensure coordinated patient care and reduce hospital readmissions. Maintain strict compliance with all relevant regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Oversee pharmacy inventory management, including refrigerated products, and prepare medications and supplies for secure and prompt home delivery.
Qualifications and Desired Experience:
Minimum of 2 years experience in infusion specialty pharmacy. Familiarity with IVIg therapies is highly desirable. Strong knowledge of specialty therapy payer requirements, reimbursement procedures, and contract obligations. Current and in good standing NY pharmacist license. Experience with CareTend system preferred.
