Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A clinical pharmacist specializing in home infusion is responsible for preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients in a home care setting. Acting as the central coordinator in a multidisciplinary healthcare team, this role ensures safe sterile compounding, adjusts medication dosing based on clinical data, and provides essential education to patients and caregivers for effective treatment management.
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions within a sterile, ventilated environment in compliance with state and federal regulations. Assess laboratory values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatment. Deliver detailed counseling and education to patients and caregivers on infusion pump usage, sterile techniques, and catheter care at home. Collaborate closely with prescribing physicians, home health nurses, and internal teams including intake, referral, reimbursement, clinical services, and pharmacy operations to ensure coordinated patient care and prevent hospital readmissions. Ensure compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards. Manage pharmacy inventory, including refrigerated products, and prepare medication shipments for timely delivery to patients.
Qualifications:
Minimum 2 years of infusion specialty pharmacy experience. IVIg experience considered a plus. Knowledge of specialty therapy payer requirements, reimbursement procedures, and contract obligations. Current and good-standing New York pharmacist license. Experience with CareTend system.
