Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire A healthcare organization is seeking a skilled Clinical/Infusion Pharmacist to support their Syracuse, NY operations. This direct hire opportunity requires a licensed pharmacist with specialized experience in home infusion and IV medication management. The position offers a Monday to Friday schedule from 8:00 a.m. to 4:30 p.m. and includes participation in an on-call rotation to ensure continuous patient care.

Responsibilities:
Prepare and dispense sterile IV and subcutaneous medications in compliance with all state and federal regulations. Perform clinical monitoring, including assessment of lab values, pharmacokinetic dosing (e.g., vancomycin), and Total Parenteral Nutrition (TPN) management. Provide patient and caregiver education on infusion pump use, sterile techniques, and catheter care. Collaborate with physicians, home health nurses, and interdisciplinary teams to ensure coordinated therapy and reduce hospital readmissions. Maintain regulatory compliance with agencies such as the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigerated medications, and oversee the secure packaging and delivery of supplies to patients' homes.
Desired Qualifications:
Minimum of two years' experience in infusion specialty pharmacy. Familiarity with IVIg therapy is an advantage. Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations. Experience with CareTend system preferred. Current and active pharmacist license in New York in good standing.
